"Assertion" essentially says "the semantics of the question are expressed in the value attribute". It's frequently used with SNOMED-CT where the code sent in the value is something like "allergic to x". In those cases,it's redundant (and sometimes dangerous) to specify an Observation.code of something like "allergy". So we fix the value to the generic "assertion" and the value attribute contains what's being asserted (allergic to x,diagnosed with y, etc.) Full discussion can be found in the TermInfo material
