Abstract

Causes of accidents, or maybe better determinants of exposure to hazard(s), is a rather complex field. Not only physical barrier breaching is at stake, but also decision making, leading to the design of machines, installations and barriers. The design itself can be a source of hazards, and possible process deviations too. And finally wrong actions of people who are either executing tasks or bare the responsibility. To understand causal relations between these determinants and the occurrence of accidents is quite a rather knotty problem, and reveals the complexity safety science. This paper will discuss the first steps taken to understand these determinants, starting at a period long before the industrial revolution, till the interbellum, the period between World War I and II.
